All people aged 45-49 who are at risk of a chronic condition are eligible for a health assessment. Aboriginal and Torres Strait Islanders and patients over the age of 75 are also encouraged to complete an annual health assessment with our Practice Nurse and GP. All health assessments are fully funded if you are eligible and have a Medicare card.
